Tulip Tree Family Health Care is staffed by Family Nurse Practitioners who are educated in primary care areas ranging from preventative medicine to treatment and dedicated to offering a full range of medical services. They strive to offer education to each patient to help them build and maintain an overall healthy lifestyle in as comfortable and affordable situation as possible. We also offer well-visits for infants and children, immunizations, acute sickness visits, physical exams, female exams, flu vaccine clinics and limited school/sports exams.

Laboratory Services 

Need to find a local Quest Diagnostics lab draw site?  You don't need to look any further!  We offer walk-in hours or appointments Monday through Thursday from 8 am to 12 pm and 1 pm to  4 pm and Fridays we are currently CLOSED.  We also have additional times available by appointment.  Results always go directly to your physician.  Have a question?  Just give us a call!

Dental Services

We understand there is a great need for dental services in our area and we are here to help!  Whether you need emergency or routine dental treatment, call today to see if you qualify and get program details!  Can't find a dentist who accepts Indiana or Illinois Medicaid - we do!
 

Pregnancy Testing

Might be pregnant?  Have some other questions?  Confidential and discreet, we offer free pregnancy tests and would be happy to answer any questions you may have.
 

Health Education

Education sessions are available to our patients for a variety of topics including smoking cessation, nutrition, and diabetic care.

We are currently offering nutrition classes for our patients.  These are being offered one day a week so please call the office to sign up.

Sliding Fee Scale Discount Program

In offering affordable healthcare to our community, we are pleased to be able to provide the Sliding Fee Discount Program to those individual/families who qualify. This program allows us to discount our fees and services based on the total household income. Educational Model Lab

Medical Students and Educators can use the models now available for study time and educational presentations.  Students may walk in during regular business hours to study and Educators may check models out.  If you have any questions, please call the office and ask for Melanie or Martha.
