We provide these services for individuals and families both with and without insurance. 

For those without insurance, we offer an Income-Based Sliding Fee Scale.  For those with insurance, we accept most private insurances plans in additiona to Medicare, Medicaid, and Indiana HIP insurance.  Whether you need direct patient care, health education or information for our area, we are here for you.
