Who is eligible for the Sliding Fee Discount Program?

Anyone who does not currently have health and/or dental insurance is eligible to apply for the Sliding Fee Discount Program.  Discounts are based on household income and are unique to each household situation.   The best and most accurate way to determine what your individual discount would be is to call the office and ask to speak to an Intake Coordinator.

How do I sign-up or enroll?

Call the office at 812-753-1039 and ask for an Intake Coordinate who will schedule your personal administrative appointment to calculate your discount and complete your paperwork.

How soon can I be seen for treatment?

As soon as your paperwork has been completed and discount has been calculated, you will be able to schedule the first available appointment.
 

I don't have any income at all, so I won't have to pay anything, right?

Unfortunately, we are not a free medical or dental clinic.  However, almost every person who completes their administrative appointment is eligibile for a discount and we offer VERY affordable payment arrangements tailored to your individual situation.  The best, and really the only way to fully understand our program is to complete an administrative appointment.  Call the office today to schedule yours!
